At Meridian Gardens, we customize our programs to
provide just the right amount of assistance to foster
independence and provide enough supportive services to
ensure our residents maintain and/or enhance their
quality of life. It is important to us that each
resident live with dignity and be treated with the
utmost respect.
Medical Services

Resident service levels
are provided for the most individualized and
professional service. An in-depth evaluation tool has
been developed to determine each resident's level of
care.

Our Registered Nurse
partners with each resident, their family and
physician to develop a plan of care unique to meet
each person's needs and wants.

The Resident Service
levels are based on a point system with respective
monthly charges.

Base Level - 0 points
Level 1 1 9 points
Level 2 10 19 points
Level 3 20 29 points
Level 4 30 39 points
Level 5 40 49 points
Level 6 50 59 points

Assessments are done at
the following times: upon admission, when changes
occur in resident's healthcare status, and annually.

Apartment Services
-
Five floor plans are
available for selection: studio, one bedroom, one
bedroom with a den, two bedrooms with one bath, and
two bedrooms with two baths.

Meridian Gardens has 60
apartments and is licensed for occupancy of 80 people.
-
The facility features
three stairwells and an elevator to accommodate first
and second floor apartments.

Apartments with
interior, outside and courtyard views are available
without differential pricing.

Rents are collected on a
month-to-month basis and do not require long-term
contracts.

Resident Services
-
Facility amenities are
provided to give our residents more convenience and
choices in their lifestyle. Fashioned after a town
square' theme, our assisted living community features
a library, theatre, coffee shop, chapel, beauty/barber
shop, whirlpool and wellness room, plus three distinct
lounges.

Various religious
services and Bible studies are provided onsite in the
Chapel.

Beauty shop and
barbering services are provided in the building by
appointment. Manicures and pedicures are among the
variety of services offered.

Stop in at the coffee
shop for complimentary ice cream, popcorn, and coffee
while hearing tunes on the juke box.

Activities are tailored
by our Life Enrichment Coordinator to appeal to our
resident's interests: crafts, games, cards,
woodworking, current event reviews, outside
entertainment and van outings.

Residents may bring pets
up to 35 lbs to live with them. A $1,000
non-refundable deposit is required.

Whirlpool bathing is
available in our unique no-lift tub with side entry
for relaxation and comfort.

Professional management
and staff are available 24-hours each day to provide
assistance with hospitality and daily living needs.

Meal Services

Three nutritious meals
are served restaurant style in our dining room.
Residents exercise independent choices in meal
selections.

A private dining room is
available for reservation by residents to enjoy
special meals with family and friends.

Housekeeping and Laundry Services

Weekly housekeeping is
provided.

Basic laundry services
include changing and washing bedding and flat linens.

Meridian Gardens
features two onsite laundry rooms. Residents may
choose to do their own laundry, have their family do
it, or may request staff do it as an optional
selection under resident service level of care.

Transportation and Maintenance
Services
-
Transportation is
available by the facility to medical appointments and
for special requests. A van with a wheelchair lift is
offered.

Complete maintenance of
building and grounds is provided. This includes snow
removal, trash pickup, and personal vehicle
assistance.

Regular maintenance and
repair of apartment appliances is provided.

Security Services
-
Meridian Garden's main
entrance is key pad entry which offers residents and
their families independence to come and go, but
security monitoring of other individual traffic.
-
Facility exterior doors
have alarms to alert staff by pager when these doors
are opened.
-
All rooms have smoke
detectors, sprinkler units, and fire alarms. Monthly
fire drills are conducted for staff and residents.
-
A Register Nurse is
on-call at all times. 24-hour certified staffing is
present on-site to monitor health and medication
needs.
-
Each resident wears an
emergency code alert unit to notify staff of
individual needs or emergencies.
